Job Application Form

Short description

A complete, professionally structured job application form for hotel recruitment, organised into nine numbered sections covering everything from the position applied for through to a signed declaration and data consent clause. Form coded, revision controlled, with an attachment checklist and an HR screening block, built to sit alongside the Effective Recruitment Procedure SOP as its candidate facing document.

Purpose

A curriculum vitae tells you what a candidate chose to tell you. An application form tells you what you need to know. The difference matters in hospitality, where availability for shifts, willingness to be placed across properties, notice period, and expected salary decide whether a candidate is viable long before their experience does, and none of those appear on a typical CV.

This form does four things:

  1. It makes candidates comparable. Every applicant supplies the same information in the same order, so screening becomes a consistent exercise rather than an interpretation of twenty differently formatted documents.
  2. It captures the screening deciders early. Expected salary, available start date, notice period, shift willingness and placement flexibility are all on page one or close to it, so an unviable application is identified in a minute rather than after two interviews.
  3. It collects what HR will need later anyway. ID card number, tax ID, BPJS number, emergency contact and reference details are gathered at application, which means the eventual hire’s personal file is most of the way complete before day one.
  4. It protects the hotel legally. A signed declaration confirms the information is true, acknowledges the consequences of false information, and grants explicit consent for reference verification and personal data use under applicable data protection law.

It suits hotels of any size formalising their recruitment intake, HR departments replacing an inconsistent CV only process, properties recruiting at volume during pre opening or peak season, and any operation that needs a defensible written record of what a candidate declared at the point of application.

What is inside the document

A single structured form in nine numbered sections, with a header block, an attachment checklist, and an internal screening block.

1. Form header

A header carrying the hotel name, form title, form code HRD/FRM-REC/02, revision number, and department, alongside a marked space for a 3 by 4 centimetre photograph. A completion instruction sits in the header stating that the form is to be completed in English using capital letters, and that fields marked with an asterisk are mandatory. Required field marking appears throughout, which measurably reduces incomplete submissions.

2. Section 01, Position Applied For

Six fields establishing viability immediately, covering the position, department, expected monthly salary, available start date, employment type across permanent, contract, daily worker and internship, and the sourcing channel through which the candidate found the vacancy.

That last field quietly produces a useful dataset. After fifty applications, it tells HR which channels actually deliver candidates, which is exactly the evidence needed to justify or cut recruitment spend.

3. Section 02, Personal Information

Eight fields covering full name as stated on the identity card, nickname, place and date of birth, nationality, identity card number, tax identification number, height and weight, and BPJS number.

4. Section 03, Contact Details

Six fields covering mobile and WhatsApp number, email, current address, and a mandatory emergency contact with name, relationship and telephone number. Making the emergency contact mandatory at application stage is sound practice, since it is otherwise collected late or not at all.

5. Section 04, Education

A three row table across six columns, covering high school or vocational, diploma, and bachelor or higher, each recording the institution, major, city, dates attended, and grade or score.

6. Section 05, Work Experience

A three row table across six columns, covering the company or hotel, position held, dates, last salary, main duties, and reason for leaving, with an instruction to begin with the most recent position and to attach a separate sheet if required.

The reason for leaving column is the single most useful field on the form. It is asked once in writing, at a moment when the candidate is answering carefully, and it gives the interviewer something concrete to explore later.

7. Section 06, Languages, Skills and Training

Three blocks covering language ability, rated separately for speaking, writing and reading across basic, good and fluent, with English listed and space for a second language; a training and certification table recording the programme, organiser and year; and a skills block covering computer literacy with hospitality specific options including Opera PMS and POS systems, plus free text for other skills.

Naming Opera PMS and POS systems explicitly rather than asking generically about computer skills is the kind of detail that only someone who has recruited for hotels would include.

8. Section 07, References

A two row table capturing the referee name, position and company, relationship marked as former supervisor or other, and contact details. Requesting two referees at application stage saves a chasing exercise later, and directly supports the two referee minimum set out in the Recruitment SOP.

9. Section 08, Additional Information

Seven questions covering the operational realities that decide fit in hospitality, namely willingness to work shifts, weekends and public holidays, willingness to be placed in another department or property, whether the candidate has relatives working at the hotel, whether they have previously applied or worked there, any health condition affecting the role applied for, the notice period at their current employer, and a free text question on why they want to join.

The relatives question supports the conflict of interest rules set out in the Recruitment SOP, and the previous application question prevents the awkward situation of unknowingly re interviewing a candidate rejected six months earlier.

10. Section 09, Declaration and Consent

A written declaration confirming that all information is true and complete, acknowledging that false information may lead to cancellation of the application or termination of employment, and granting consent for reference verification and for the use of personal data for recruitment purposes only, in line with applicable personal data protection law. Signed with place, date, signature and printed name.

11. Attachment checklist and HR screening block

A six item documents checklist covering CV, identity card copy, latest diploma and transcript, employment certificates, training certificates and a recent photograph, followed by an HR use only block recording the date received, who screened the application, the outcome across Shortlist, Talent Pool or Rejected, and remarks.

The Talent Pool outcome is a small but deliberate touch. It means a capable candidate rejected for one vacancy is recorded as a future prospect rather than lost, which is precisely what the Recruitment SOP requires.
